Conditions A to Z

Gynaecological Conditions

Gynaecology encompasses the branch of medicine concerned with the health of the female reproductive system — including the uterus, ovaries, fallopian tubes, cervix, and vagina. It covers an enormous range of conditions, from the relatively common and benign to the serious and complex. Gynaecological health is intimately connected to hormonal balance, and many conditions are influenced by the hormonal changes that occur throughout a woman's life — from puberty through to the menopause and beyond.

Common conditions

Some of the most frequently encountered gynaecological conditions include menstrual disorders (such as heavy, irregular, or painful periods), polycystic ovary syndrome (PCOS), endometriosis (where tissue similar to the uterine lining grows outside the uterus), uterine fibroids (non-cancerous growths of the uterine muscle), ovarian cysts, pelvic inflammatory disease (PID), and prolapse of the pelvic organs. Gynaecologists also manage vulval conditions, vaginal infections, and cervical changes detected through cervical screening.

Symptoms that may prompt investigation

Symptoms warranting gynaecological assessment include abnormal uterine bleeding (whether between periods, after sex, or after the menopause), pelvic or lower abdominal pain, unusual vaginal discharge, painful intercourse, difficulty conceiving, and urinary symptoms such as incontinence or recurrent infections.

Diagnosis

Assessment typically includes a detailed medical history, pelvic examination, and investigations such as pelvic ultrasound, blood tests for hormonal levels or tumour markers, cervical smear tests, and in some cases hysteroscopy (examination of the inside of the uterus with a small camera) or laparoscopy.

Treatment

Treatment varies enormously depending on the specific condition. Hormonal therapies — including the contraceptive pill, progesterone preparations, and hormone replacement therapy — play a central role in managing many gynaecological conditions. Surgical options, including minimally invasive laparoscopic procedures, are available for conditions such as endometriosis, fibroids, and ovarian cysts. Infectious conditions are treated with appropriate antimicrobials.
